بررسی مقایسه‌ای تاثیر دگزامتازون وریدی با بودسوناید استنشاقی در کاهش استریدور بعد از اکستوباسیون در بیماران ICU

Background and Objective: Stridor is considered as a serious adverse complication of intubation that may lead to respiratory distress, reintubation, and further complications. Intravenous steroids including dexamethasone are assumed as an effective therapy. However, their associated side effects pose a limit on their usage. This study was carried out to elucidate the effect of intravenous dexam...

Pre-emptive antibiotic therapy to reduce ventilator-associated pneumonia: “thinking outside the box”

Mechanically ventilated, intubated patients are at increased risk for tracheal colonization with bacterial pathogens that may progress to heavy bacterial colonization, ventilator-associated tracheobronchitis (VAT), and/or ventilator-associated pneumonia (VAP). Previous studies report that 10 to 30 % of patients with VAT progress to VAP, resulting in increased morbidity and significant acute and...

Aerosolized antibiotic therapy: a forward aerial assault or forced contingency for ventilator-associated pneumonia?

The increasing prevalence and complexity of multipledrug-resistant (MDR) organisms in nosocomial infections have fueled interest in novel therapeutic approaches. Adjunctive aerosolized antibiotic (AAA) therapy for pneumonia has gained popularity, supported by apparent advantages of higher pulmonary antibiotic concentrations and lower systemic exposure. Balanced by a paucity of highlevel evidenc...

Aerosolized colistin for the treatment of nosocomial pneumonia due to multidrug-resistant Gram-negative bacteria in patients without cystic fibrosis

INTRODUCTION The clinical and economic consequences of the emergence of multidrug-resistant Gram-negative bacteria in the intensive care unit (ICU) setting, combined with the high mortality rate among patients with nosocomial pneumonia, have stimulated a search for alternative therapeutic options to treat such infections. The use of adjunctive therapy with aerosolized colistin represents one of...

The use of inhaled antibiotic therapy in the treatment of ventilator-associated pneumonia and tracheobronchitis: a systematic review

BACKGROUND Ventilator-associated respiratory infections (tracheobronchitis, pneumonia) contribute significant morbidity and mortality to adults receiving care in intensive care units (ICU). Administration of broad-spectrum intravenous antibiotics, the current standard of care, may have systemic adverse effects. The efficacy of aerosolized antibiotics for treatment of ventilator-associated respi...
